About Challenge
The Healthcare InnoMatch 2022 by Temasek Foundation and the Centre for Healthcare Innovation (CHI) is calling for innovative, market-ready or near market-ready solutions to address critical and emerging healthcare needs!Organised in partnership with SingHealth Service (SHS), National University Health System (NUHS) and National Healthcare Group (NHG), the Healthcare InnoMatch 2022 is seeking solutions that can enhance patient care and deliver positive health outcomes with the theme: Delivering Care Beyond Hospital Walls. This encompasses solutions that bridge the gap between hospital and home/community; including preventive and predictive care, transition of care, and patient activation for better self-management of care.
Successful proposals in the Healthcare InnoMatch 2022 will obtain funding support to demonstrate their solutions in operational environments, and will receive further endorsement from our partner healthcare clusters upon successful validation of the results of pilot implementation. Ultimately, the platform aims to bridge the market adoption gap, and allow patients early access to new and emerging healthcare innovations to address our critical healthcare needs.

The future hospital is one without walls, where care follows the patient. As care continues after hospitalisation, the transition of care from the acute hospital into the community should be done in a seamless manner. In such a relationship-based person-centric care model, it is important to develop care plans that motivate long term behaviour change, build resilience and cultivate ownership of one’s health to help patients manage their conditions effectively outside their regular clinical visits.
How can digital transformation technologies and innovation solutions enable a smooth transition of care and greater self-management to better promote and maintain health, prevent diseases, and live with illness and disability at home well while keeping cost affordable and sustainable in the community?
What We Are Looking For
Solution may encompass the following:
- Bridge the gap between hospital and home/community (both upstream and downstream).
- Provide timely intervention to monitor and manage patients remotely.
- Deliver solutions for preventive and predictive care, transition of care, and patient activation for better self-management of care.
